Caris Healthcare created Caris Transitions to streamline communication and ensure patients receive hospice services when appropriate.
We recognize a call to Caris is a call for help. If we receive a referral from a physician and the patient is not eligible or ready for hospice services, we continue to be a resource for the patient and family through Caris Transitions.

Caris Healthcare checks in with Caris Transitions patients based on patient and family preference. We communicate patient and family decisions with the referring physician to keep everyone informed.
